I Have Already Received My COVID Vaccine, but Have Not Submitted My Proof of Vaccination

If you have already received your COVID vaccine, submit your proof of vaccination to the Wolfie Health Portalas soon as possible, but no later than Friday, September 24, 2021.  You must upload your proof of vaccination by September 24 to remain enrolled in your in-person fall classes and/or use  an in-person campus service or facility . Proof of vaccination of a two-part series is the completion of both doses. Click here for instructions on how to upload your proof of vaccination to the Wolfie Health Portal.
